Outreach therapy means receiving specialist rehabilitation outside the hospital or clinic — in the patient’s home, care environment, school, workplace or community setting.
It is ideal when recovery depends on practising real tasks in the real environment: transfers, stairs, kitchen activities, mobility outdoors, car access, fatigue pacing, communication, family routines and safe independence at home.
